Privacy Policy

Your information,handled with care.

Effective September 25, 2026

This Privacy Policy explains what personal information Restore Clinic LLC (“Restore Clinic,” “we,” “us,” or “our”) collects, why we collect it, who we share it with, and the choices you have. It covers this website and the everyday handling of information for consultations, appointments, and treatment.

We keep this simple on purpose: we collect what we need in order to answer you and care for you well, we do not sell it, and we do not use your information to advertise to you or to anyone else.

Information You Give Us

Most of the information we hold is information you chose to give us.

Through the website contact form

When you request a consultation, the form asks for:

  • Your name.
  • Your email address.
  • Your phone number, which is optional.
  • The treatment you are asking about.
  • Whatever you choose to tell us about the area you would like to address.

That message is delivered to the clinic as an email. It is not published, posted, or added to an advertising list.

Please share only what you are comfortable putting in an email. The form is for starting a conversation — it is not a secure channel for detailed medical history, identification documents, or clinical photographs.

When you call, email, or text us

We keep the contact details and the substance of what you tell us so that we can pick the conversation back up where it left off.

When you become a client

Before treatment we collect the information required to treat you safely and lawfully, which may include health history, medications, allergies, prior treatment of the area, informed-consent and photography forms, appointment records, and payment records.

Information Collected Automatically

Like nearly every website, this site is delivered by a hosting provider that keeps standard server logs. Those logs may include an IP address, browser and device type, the pages requested, and the date and time of the request. They exist for security, error diagnosis, and keeping the site online.

This site does not run advertising pixels, does not build visitor profiles, and does not track you across other websites.

Our online booking is provided by Mangomint. Its booking script loads on every page so that a booking window can open without sending you to another site, which means Mangomint receives the standard technical request data described above. It is used to operate booking, not to advertise to you.

Cookies

Restore Clinic does not use advertising or analytics cookies on this website, and there is no advertising profile built from your visit.

When you open the booking window, our booking provider, Mangomint, may set cookies or similar storage that are necessary to run a booking — to keep your selections together and complete the appointment. These are operational, not advertising, and Mangomint handles them under its own privacy policy.

Your browser may still store ordinary technical data needed to load and display pages. You can clear or restrict that at any time in your browser settings without losing access to the site.

If we later add website analytics, a payment, or a chat tool that sets cookies, this section and the effective date above will be updated before that tool goes live.

How We Use Your Information

We use personal information to:

  • Answer your enquiry and arrange a consultation.
  • Assess candidacy, plan treatment, and provide care safely.
  • Send appointment confirmations, reminders, preparation instructions, and aftercare instructions.
  • Take payment and keep accurate financial and treatment records.
  • Respond to questions, concerns, or a request about a result.
  • Meet legal, insurance, licensing, tax, and record-retention obligations.
  • Keep the website secure and working.

We do not sell personal information. We do not rent, trade, or share it with data brokers, and we do not use client health or treatment information for advertising.

Treatment Information and Confidentiality

Restore Clinic provides cosmetic and paramedical services on a cash-pay basis and does not bill health insurance, so we are generally not a HIPAA covered entity. We say that plainly rather than implying a protection that does not apply.

Your health history, treatment records, and images are nonetheless treated as confidential. We handle them under our own practice policy and applicable Arizona law, we limit access to the provider and any staff who need it to do their work, and we do not discuss your treatment with anyone outside the clinic without your permission — except where the law requires disclosure.

Photographs

Photographs of a treatment area are a normal part of assessing skin and documenting healing, and they are treated as part of your confidential record.

Clinical photographs are taken and kept for your treatment record by default.

We will not use your photograph on the website, on social media, in marketing, or in any portfolio unless you have signed a separate photography release specifically permitting it.

A photography release is voluntary. Declining it does not affect the treatment you receive or the price you pay, and it is not a condition of being treated.

You may withdraw permission for future marketing use at any time by contacting us. We will stop using the image going forward, though we may not be able to recall material already printed or already shared by others.

Please do not send clinical photographs with a first enquiry. Once we reply, we will tell you what kind of image is useful and how to send it.

Who We Share Information With

We share personal information only where it is necessary, and only with the categories below:

  • Service providers who operate the tools we rely on — currently our website host, the email delivery service that carries contact-form enquiries to us, and Mangomint, which provides our online booking and appointment system. They may process information only to provide that service to us.
  • Payment and financing providers, when you choose to pay or finance through them. They collect and handle your payment details directly under their own terms and privacy policies. Restore Clinic does not store full card numbers.
  • Professional advisors such as our attorney, accountant, or insurer, where genuinely needed.
  • Another healthcare or medical provider, when you ask us to or agree to it — for example, when an area should be evaluated by a physician.
  • Law enforcement, regulators, or a court, where the law requires it or where it is necessary to establish or defend a legal claim.

If the practice is ever sold or transferred, client records may transfer with it. You would be notified, and this policy would continue to apply to information collected under it until it is replaced.

Email, Text Messages, and Marketing

When you give us your contact details, you are authorizing us to use them to reply to you and to send messages about your appointment, consultation, payment, or treatment.

Marketing or promotional messages are sent only with the consent the law requires, and every one of them includes a way to stop receiving them. You can also simply ask us to stop, and we will.

Opting out of marketing does not stop appointment and care messages, because those are part of treating you. Standard carrier or data rates may apply to text messages.

Email and text are not secure or private by nature. Please do not use them for emergencies — if you have a medical emergency, call 911 or seek emergency care.

How Long We Keep Information

We keep information for as long as it serves the purpose it was collected for, and then for as long as the law, our insurer, or a licensing requirement obliges us to keep it.

Treatment, consent, and financial records are kept for the period required of a practice of this kind in Arizona.

Enquiries that never become appointments are kept only as long as they are useful for following up, and are then deleted.

Records we are required to retain cannot be deleted on request until that retention period has ended.

How We Protect Information

We take reasonable, practical steps to protect personal information: access is limited to people who need it, accounts are individually secured, devices and files holding client records are protected, and paper records are stored securely.

No website, email system, or record system can be guaranteed completely secure, and we will not claim otherwise. If a breach affecting your personal information occurs, we will notify you as required by applicable law.

Your Choices and Rights

You are always free to:

  • Decline to give information, understanding that some information is necessary in order to treat you safely.
  • Ask what personal information we hold about you and request a copy of your record.
  • Ask us to correct information that is inaccurate or out of date.
  • Withdraw a photography release, or any other optional consent, for future use.
  • Opt out of marketing messages at any time.
  • Ask us to delete information that we are not legally required to retain.

To make any of these requests, contact us using the details at the end of this policy. We may need to verify your identity before releasing or changing a record, and we will respond within the time applicable law allows.

Depending on where you live, you may have additional statutory privacy rights. Tell us which state you reside in when you make a request, and we will honor the rights that apply to you.

Children's Privacy

This website is intended for adults, and treatment clients must be at least 18 years old unless Restore Clinic has expressly authorized a lawful exception in writing.

We do not knowingly collect personal information from children. If you believe a child has submitted information through this site, contact us and we will delete it.

Links to Other Websites

This site may link to social media, mapping, financing, or other third-party services. Those services are operated by other companies under their own privacy policies, and this policy does not govern what they collect once you leave our site.

We would encourage you to read the privacy policy of any financing or payment provider before submitting information to it.

Changes to This Policy

We may update this Privacy Policy as the practice, the website, or the law changes. The effective date shown at the top of this page identifies the current version.

Changes apply from the date they are published. Where a change is significant, we will make reasonable efforts to bring it to the attention of current clients.

Contact Us

Questions about this policy, about the information we hold, or about a privacy request can be sent through the contact form or to the contact details published on this website.

If your question concerns a specific record, including your full name and the approximate date of your appointment helps us find it quickly.

Questions about your privacy?

Ask us before you share anything you are unsure about.

If you would rather discuss something by phone than put it in a form, that is completely fine — call us and we will take it from there.
